Alpha-GPC: choline on a glycerophosphate backbone

Alpha-GPC (L-alpha-glycerylphosphorylcholine, also sold as choline alfoscerate) is a choline molecule attached to a glycerophosphate backbone. It is water soluble, it occurs naturally in the body and in small amounts in food, and a large share of each gram is choline itself, one of the highest fractions of any common supplement form. Both compounds are studied as acetylcholine precursors, meaning research treats them as raw material the body can use toward the neurotransmitter acetylcholine.

Citicoline: choline plus cytidine, in one molecule

Citicoline (CDP-choline, cytidine diphosphate-choline, or the branded form Cognizin) is a two-part structure: choline linked to cytidine. Because cytidine occupies part of the molecule, a smaller share of each gram is choline than in alpha-GPC. Citicoline's research tradition treats the cytidine half as interesting in its own right, which is part of why the two compounds have separate literatures rather than one shared file.

A 2025 systematic review and meta-analysis pooled all three, 358 patients in total, and found alpha-GPC improved overall clinical condition on the SCAG scale more than citicoline, with no significant difference between the compounds on memory or word-fluency tests. Split decision, small numbers.

The earliest direct comparison is also worth reading in full. In a 1991 randomized multicentre trial, 120 patients with mild to moderate vascular dementia received injected alpha-GPC or citicoline at 1 g per day for 90 days; both groups improved on symptom measures and alpha-GPC scored statistically higher on most tests, but there was no placebo arm and no blinding. Choline bitartrate: the cheap third option

Choline bitartrate is choline bound to a tartrate salt: no phospholipid structure, no cytidine, usually the lowest price per gram of choline on the shelf, and the form most multivitamins and blends quietly use. Two facts from the verified library frame it fairly. First, in a 2022 randomized crossover in six healthy men, choline chloride, choline bitartrate, glycerophosphocholine, and egg phosphatidylcholine all raised plasma choline to a similar degree at matched choline doses, while every water-soluble form, glycerophosphocholine included, also raised TMAO, a gut-derived metabolite that cardiovascular researchers are still working out.

Second, the older clinical literature is less flattering: a 2001 review of cholinergic precursors reported that plain choline and lecithin failed in controlled dementia-era trials, while CDP-choline and alpha-GPC produced modest cognitive improvements in that clinical context. In a 2025 cohort of 508,107 patients newly diagnosed with mild cognitive impairment, alpha-GPC users showed a lower risk of progressing to dementia and lower stroke risk among those who did not progress, also observational, also unable to prove causation.
