Nobody knows yet. In mice, allicin from raw garlic lowered TMAO, and one week of raw garlic juice reduced TMAO formation in a small group of screened human volunteers. No human trial has tested any garlic extract on TMAO, and no study has combined garlic with a choline supplement. That bridge is a hypothesis.

Why would a choline supplement create TMAO in the first place?

Start with the chain, because every claim on this page hangs off it. Step one: you swallow choline, an essential nutrient, and alpha-GPC is a choline compound whose job is delivering it. Step two: choline that is not absorbed early in digestion reaches gut bacteria, which convert part of it into TMA, trimethylamine. Step three: liver enzymes oxidize TMA into TMAO, trimethylamine N-oxide, which circulates in blood until the kidneys clear it. That relay is textbook biochemistry, and it frames a 2026 narrative review that weighs the molecule's uses against the TMAO question and notes the TMAO-harm literature itself is inconsistent (PMID 42196986, review).

The alpha-GPC entry point has even been mapped: in cell and mouse experiments, an intestinal enzyme, Gpcpd1, clips supplemental GPC into free choline in the gut, and deleting it in mice partially blocked the TMAO rise after a GPC dose (PMID 39510189, animal and cell data, not human).

Does an alpha-GPC dose actually raise TMAO in people? It can: two small randomized crossovers in healthy men, 6 each, measured a rapid TMAO rise after oral alpha-GPC (PMIDs 34287673 and 36840817, human randomized crossovers), while a third found no significant rise after a single 480 mg dose in 12 men (PMID 38490741, human randomized trial). The reason anyone cares: in observational research, higher circulating TMAO tracks with higher cardiovascular risk. Association, not proven causation, and that association is why the garlic idea exists.

Does garlic lower TMAO? Every study that exists

The garlic-TMAO idea traces to one specific paper, and the paper is both better and smaller than the way it gets quoted.

The anchor. In 2022, researchers published a multi-part study built around allicin, the sulfur compound crushed raw garlic is known for (Panyod 2022, animal-primary study). In mice fed carnitine, another TMA precursor, allicin supplementation decreased serum TMAO. In carnitine-fed, plaque-prone ApoE-knockout mice, it reduced aortic lesions. And in the part everyone remembers: a small group of humans, screened first to confirm they were high TMAO producers, drank raw garlic juice for one week, and their TMAO formation dropped.

Read that human arm again, because it carries the entire internet claim. One week. Raw garlic juice, not a capsule or an extract. Volunteers pre-screened as high TMAO producers, which the general population is not. It is an exploratory arm inside a mouse-primary paper, and it is the only human garlic-TMAO evidence in PubMed: a targeted search for garlic and TMAO in humans returns three papers, this one, a mouse study, and a review.

The supporting cast is animal-only. Aged garlic oligosaccharides, a carbohydrate fraction distinct from the commercial aged extract, lowered TMAO in plaque-prone mice on a high-fat diet (PMID 39326312, animal study). And in healthy rats, allicin lowered urinary TMAO, with a catch that rarely makes the retelling: the allicin was injected into the abdomen, not swallowed (PMID 20055178, animal study).

Now the two sentences this page exists to say plainly. No human trial of any garlic extract, capsule, powder, or tablet has ever measured TMAO. And no study anywhere has combined garlic with a choline supplement, alpha-GPC included, and measured what happens to TMAO. The bridge between the garlic literature and the choline literature is a mechanistic hypothesis: plausible, early, worth studying, and not human-proven.

Which garlic? The form problem nobody prints on the label

Here is the part that quietly deletes most of the shortcut logic: garlic is not one substance. The TMAO findings above belong to allicin, which is famously unstable: it forms only when crushing brings an enzyme together with its substrate, and it degrades quickly. How much allicin a product actually delivers depends on processing, and the measured spread is enormous.

A 13-person crossover measured allicin bioavailability from 13 supplements and 9 garlic foods against crushed raw garlic, using a breath marker (Lawson 2018, human crossover). Black garlic delivered 5 percent of raw garlic's allicin. Boiled garlic, 16 percent. Enteric-coated tablets ranged from 36 to 104 percent, and dropped to 22 to 57 percent when taken with a high-protein meal.

The tablet story gets worse when you test the tablets themselves. USP dissolution testing of all 24 known brands of enteric-coated garlic powder tablets found 83 percent released under 15 percent of their labeled allicin potential under simulated digestive conditions, even though the same tablets met their label claim when crushed (PMID 11368641, laboratory dissolution study). Consistency within one product is not safe to assume either: ten lots of a single leading allicin-standardized brand varied threefold in released allicin across production years, 44 percent of potential versus 15, while a second brand used in a negative trial released essentially none (PMID 11270714, laboratory dissolution study).

Aged garlic extract sits at the other pole. It is processed so that almost no allicin remains, its marker compound is S-allylcysteine, SAC, and its human evidence base, covered below, was earned at defined SAC doses. Aged-extract findings do not transfer to raw garlic, and raw-garlic findings, the TMAO arm included, do not transfer to aged extract.

So a label that reads only garlic extract, with no form and no marker compound, has told you almost nothing: it cannot point to the raw-juice TMAO arm, to the aged-extract trials, or to the allicin findings, because it cannot show it matches any of them. The table below is the whole argument.

What does garlic actually have human evidence for?

After two sections of not proven, here is what garlic has actually shown in people, nulls included.

Blood pressure, in people whose blood pressure is elevated. A 2008 meta-analysis of 11 placebo-controlled randomized trials found systolic pressure 8.4 mmHg lower than placebo in the high blood pressure subgroup (PMID 18554422, meta-analysis), and the 2016 update, 20 trials and 970 participants, landed nearby: 8.7 mmHg systolic in that group (PMID 26764326, meta-analysis). A dose-response randomized trial of aged garlic extract in 79 patients with uncontrolled high blood pressure put numbers on the ladder: 480 mg per day lowered systolic pressure 11.8 mmHg versus placebo over 12 weeks, 240 mg was not significant, 960 mg borderline (PMID 23169470, human RCT). Honesty requires the other side. An independent meta-analysis of 9 double-blind trials found a similar average drop but flagged high variability, a shrinking effect in the higher-quality subset, and called the evidence, in its own words, "not strong" (PMID 25239480, meta-analysis). And in 75 healthy adults with normal baselines, a 12-week randomized trial of garlic powder found no significant effect on blood pressure, cholesterol, or arterial stiffness (PMID 15522140, human RCT). The pattern: measured effects concentrate in elevated baselines and fade in healthy ones. All of this is research reporting from trials in specific populations, not a promise any supplement bottle gets to make.

Cholesterol, where the best trial said no. The landmark six-month trial randomized 192 adults with moderately elevated cholesterol to raw garlic, a powder supplement, an aged-extract supplement, or placebo at a one-clove equivalent six days a week, with each product's chemistry verified, and found no significant effect on LDL or any plasma lipid for any of the three forms (Gardner 2007, human RCT).

Coronary plaque imaging, at 2,400 mg per day. Two roughly year-long randomized trials of aged garlic extract at that dose reported slower coronary artery calcification progression in 104 people at elevated cardiovascular risk (PMID 32349742, human RCT) and reduced low-attenuation, vulnerable-type plaque on CT angiography in 55 people with metabolic syndrome (PMID 26764322, human RCT). Imaging endpoints, specific populations, one defined dose: report them as exactly that.

Is garlic safe with blood thinners or other medication?

The safety file is mostly reassuring and specifically cautious, and the specifics matter more than the vibe.

Platelets. In adults taking a liquid aged garlic extract for 13 weeks, the threshold for ADP-induced platelet aggregation roughly doubled (PMID 11053504, non-randomized human trial). A randomized dose-finding study in 34 healthy adults found aged extract at 2.4 to 7.2 grams per day moderately raised aggregation thresholds and dose-dependently inhibited platelet adhesion (PMID 11238801, human RCT). Zoom out, though, and the picture blurs: a systematic review of 12 randomized trials, 595 participants, found reduced aggregation in only 6 of the 12, with heterogeneity high enough that the authors declined to draw any firm conclusion (PMID 36222178, systematic review).

Warfarin, the sharpest worry, has the calmest data. In a double-blind pilot in monitored warfarin patients, 12 weeks of liquid aged garlic extract produced no evidence of increased bleeding and no INR destabilization (PMID 16484565, human RCT). A randomized crossover in 12 healthy men found 2 weeks of a garlic supplement did not significantly alter warfarin pharmacokinetics or response (PMID 18516070, human RCT).

The interaction that surprises people is not about bleeding. In 10 healthy volunteers, three weeks of garlic caplets halved exposure to saquinavir, an HIV protease inhibitor, with drug levels recovering only partway after a 10-day washout (Piscitelli 2002, non-randomized human trial). Garlic can induce the pathways the body uses to eliminate certain drugs, which is a different and quieter risk than the bleeding folklore.

The doctor line: if you use blood thinners or any prescription with tight margins, or you have a procedure scheduled, garlic supplements belong in a conversation with your doctor before they belong in your routine. Has anyone tested garlic and choline together?

No. Not in a supplement trial, not in a food study, not anywhere in PubMed: no study has given people a choline supplement plus garlic and measured what happens to TMAO.

So here is what honest proof would have to look like. A randomized human trial would give a choline supplement, alpha-GPC included, with and without the exact garlic form in question. It would verify the garlic's marker compound: dissolution-verified allicin delivery for a powder, stated SAC content for an aged extract. It would run for weeks to months rather than one week, in ordinary adults rather than pre-screened high TMAO producers, and it would measure TMAO as a primary outcome. That trial has not been run by anyone: not by a university, not by an ingredient supplier, and not by any supplement brand. Until it exists, every version of "garlic cancels out your choline supplement's TMAO", on any label or in any thread, is a hypothesis wearing a conclusion's clothes.

What can be said today: alpha-GPC can raise TMAO acutely in small human studies. Allicin lowered TMAO in mice, and raw garlic juice lowered TMAO formation in one small week-long human arm. Those two literatures have never met in a single experiment. The Honest Summary

Garlic blocking choline-derived TMAO is early animal science plus one week of raw garlic juice in screened humans. No garlic extract has been human-tested against TMAO, and no garlic-plus-choline trial exists. Anyone stating it as settled, in either direction, is ahead of the evidence.
