A 2000 study in the British Journal of Dermatology found topical niacinamide increased ceramide production 4.1 to 5.5-fold in cultured keratinocytes and raised stratum corneum ceramide levels in vivo, measurably reducing TEWL in dry skin subjects.8 It pairs well with ceramide moisturizers because it supports the skin's own ceramide production while topical ceramides replenish the existing deficit. Not fully, but overnight is when repair is most active. TEWL peaks in the late evening and skin temperature drops at night, aligning with natural cellular repair cycles. Applying a ceramide-rich moisturizer at night under an occlusive (petrolatum, squalane) gives barrier lipids the longest uninterrupted integration period. Some studies have shown ceramide formulations reduce TEWL measurably within 24 hours of application.3 A single night will not undo weeks of damage, but the nightly routine is the highest-leverage daily action in the repair protocol.

Not exactly, though they overlap. Sensitive skin may have a constitutional tendency toward reactivity that is partly genetic. A damaged barrier is a functional state where the stratum corneum's lipid matrix is compromised, allowing water to escape and irritants to enter more easily. Anyone, including people without intrinsically sensitive skin, can develop a compromised barrier from over-exfoliation, harsh cleansing, or environmental exposure. People with constitutionally sensitive skin frequently have chronically reduced ceramide content, making their barrier easier to tip into a damaged state.12 The repair protocols are largely the same for both.

The fastest approach is to stop everything that caused the damage, then give the barrier exactly what it needs. Immediately cut harsh actives (strong acids, retinoids, physical scrubs). Switch to a gentle, SLS-free cleanser. Apply a ceramide-rich moisturizer morning and night, ideally with an occlusive layer at night. Studies show ceramide formulations can meaningfully reduce TEWL within 24 to 48 hours. Mild disruption often stabilizes noticeably within 1 to 2 weeks of this protocol.
