How to Get Rid of Blackheads (and Keep Them Away)
True blackheads are oxidized sebum plugs in open follicles. You cannot permanently eliminate them, but consistent use of salicylic acid (BHA) and retinoids keeps them visibly clear for most people. Blackheads have an unfair reputation as a hygiene problem. They are not. They form in follicles that produce more sebum than can exit cleanly, and once that sebum oxidizes at the surface it turns dark. 01What blackheads actually are
A blackhead is an open comedone: a pore whose opening has dilated enough that the sebum and dead cell material inside it is exposed to air. That exposure triggers oxidation of the lipids and melanin in the plug, turning it dark brown or black. The color is not dirt, and washing harder does not remove it.
The medical name for the process is retention hyperkeratosis. When dead skin cells do not shed normally from the follicular lining, they accumulate with sebum and form a plug. In an open comedone, the follicular orifice stays open, so the top of the plug oxidizes. In a closed comedone (whitehead), the orifice is sealed, so the plug stays white or skin-toned.
The dark color comes from oxidation, not from being dirty. Aggressive scrubbing irritates the skin without addressing the sebum and keratin buildup at the root of the problem.
Blackheads occur most often on the nose, forehead, and chin, where sebaceous gland density is highest. They can also appear on the back, chest, and shoulders for the same reason. They are extremely common: open comedones are one of the defining lesion types in acne-prone skin, appearing across all ages and skin types.
02Blackheads vs sebaceous filaments
Before committing to a treatment plan, it is worth checking whether what you are seeing is actually a blackhead. The most common confusion is with sebaceous filaments, and treating them as blackheads will damage your skin barrier without any benefit.
Sebaceous filaments are thin grey or cream-coloured tubes that line every hair follicle on the nose. They are a normal, permanent feature of sebaceous skin. They look like a field of tiny dots when the nose is pressed flat. They cannot be eliminated; even after extraction, they refill within days. Trying to strip them repeatedly stretches the follicle opening and can worsen their appearance over time.
True blackheads are wider, more clearly dark (brown or black rather than grey), and represent a genuinely blocked pore. They are often elevated slightly at the surface and are less uniform in distribution than sebaceous filaments.

Most of the dots on the nose are sebaceous filaments, not blackheads. Over-treating them with pore strips or aggressive BHA damages the barrier and achieves nothing lasting.
03What causes blackheads
Several factors converge to create the right conditions for open comedones:
- High sebum production. Androgenic hormones drive the sebaceous glands. More sebum means more raw material to fill and eventually overflow the follicle.
- Abnormal follicular keratinization. When cells lining the follicle shed too slowly, they mix with sebum and create a plug. Retinoids address this directly by normalizing cell turnover.
- Pore size. Genetics determine baseline pore diameter. Larger pores are more visible and accumulate plugs more quickly. Pore size is not something topicals can meaningfully change, though reducing their contents makes them appear smaller.
- Humidity and occlusion. Heavy occlusive moisturizers and certain makeup formulations can accelerate follicular filling. Non-comedogenic formulations matter on oily skin.
- Diet (limited evidence). High glycaemic load diets and dairy consumption have been linked to acne in some research, but the evidence for comedone-specific effects is weaker. Worth noting, not worth obsessing over.
Hormonal fluctuations, including those during puberty, the menstrual cycle, and periods of high stress, amplify sebum production and make blackhead formation more likely.
04The ingredients that work
Salicylic acid (BHA)
Salicylic acid is the most evidence-backed over-the-counter ingredient for blackheads specifically. Its key property is lipophilicity: unlike alpha hydroxy acids, it can dissolve into the sebum inside the follicle and work on the plug from within, not just at the skin surface.2 It also functions as a desmolytic agent, disrupting the bonds between dead cells lining the follicle wall to prevent plugging.3
A 1989 crossover study found that the only group to achieve significant comedone reduction was the salicylic acid group, with the benzoyl peroxide group showing no meaningful change in non-inflammatory lesions.4 More recently, a 28-day RCT using a 2% salicylic acid plus ceramide amino acid cleanser reported open comedone counts falling by 55.41% and hydration improving by 24%.1 The Cochrane systematic review of topical agents found salicylic acid roughly equivalent to tretinoin for comedonal outcomes, though the quality of evidence was rated low-to-very-low due to trial heterogeneity.5
For daily use, 0.5% to 2% is the standard OTC range. Higher concentrations (20% to 30%) are used in professional chemical peels and are not appropriate for unsupervised home use.
Retinoids
Retinoids (retinol, retinaldehyde, and prescription tretinoin) address the root cause of comedonal acne: abnormal follicular keratinization. They normalize the rate at which follicular cells shed, preventing the pile-up that creates the plug. They are the first-line prescription treatment for comedonal acne according to AAD guidelines.6

05Building a routine that actually works
No single product is enough. Blackhead management works when the routine addresses all three parts of the problem: clearing existing plugs, normalizing cell turnover, and reducing the rate of re-filling. Here is a practical structure:
Morning: Gentle cleanser, niacinamide serum (supports oil control throughout the day), SPF. Retinoids cause photosensitivity, so morning is not the time for them.
Evening: Gentle cleanser, BHA toner or serum (2% salicylic acid, 3 to 4 times per week if starting out, daily once tolerated), retinol (2 to 3 times per week on nights without BHA, or nightly once tolerance is established). Moisturizer over both.
A few notes on layering:
- BHA and retinoids can be used on the same night by experienced users, but those new to either should alternate to manage dryness and irritation.
- Niacinamide is compatible with both BHA and retinoids and can be layered on the same evening.
- Always follow any retinoid with a moisturizer to offset barrier disruption during the adjustment period.
- Non-comedogenic formulations matter for moisturizers and sunscreens, particularly on the nose and forehead.

07Why squeezing backfires
The impulse to squeeze is understandable. A blackhead is right there, visible, and feels like it should just pop out. The problem is what happens to the follicle when you do it without proper technique:
- Rupture risk. Uneven pressure can break the follicle wall beneath the surface, releasing debris into surrounding skin and triggering an inflammatory response. An open comedone (no inflammation) can become an inflamed papule or pustule within hours.
- Post-inflammatory hyperpigmentation. Even without full rupture, trauma to the skin triggers melanin deposition. Dark marks after squeezing are common, especially on deeper skin tones.
- Scarring. Repeated trauma to the same follicle can cause permanent textural changes.
- Bacteria transfer. Hands and fingernails are not sterile. Introducing bacteria into a compromised follicle raises infection risk.

08Niacinamide, zinc, and the oil-control angle
Oil control matters to blackhead management because sebum is the primary raw material. An ingredient that reduces how much sebum enters the follicle is doing real prevention work, even if it does not feel as dramatic as a BHA dissolving an existing plug.
Niacinamide has the best-documented OTC evidence here. A pair of RCTs covering 130 subjects found that 2% niacinamide significantly reduced sebum excretion rate over 4 to 6 weeks compared to vehicle, in both Japanese and Caucasian subjects.8 The mechanism appears to involve reduced fatty acid and triglyceride production in the sebaceous gland, though it is not fully characterized.
Zinc is another ingredient with sebum-modulating properties. In a 2025 Delphi consensus of cosmetic dermatologists, niacinamide and zinc were among the most frequently recommended ingredients for oily, large-pore skin.9
